5. How to Enter
One (1) entry per person, during the Sweepstakes Period.Entry must be received by Sponsor’s designated computer during the Sweepstakes Period set forth in Rule 1. You may enter the Sweepstakes using either of the following online methods:
A) Access www.shoppersvoice.com/smksurvey to participate in the Shopper’s Voice® Tobacco Survey Sweepstakes. Type your name, street address and email address, click on the Sweepstakes rules link, read the Official Rules and click “I have read and accept the rules of this Sweepstakes”. Then, click Next, answer the survey questions in order to be entered into the Sweepstakes, and reenter your name under Electronic Signature and click “Enter”.
B) Access www.shoppersvoice.com/smksurvey to participate in the Shopper’s Voice® Tobacco Survey Sweepstakes. Type your name, street address and email address, click on the Sweepstakes rules link, read the Official Rules and click “I have read and accept the rules of this Sweepstakes”. Then, click Next, and answer at least one (1) survey question in order to be entered into the Sweepstakes, and reenter your name under Electronic Signature and click “Enter”.
6. Drawings/Prizes / Odds
There will be one (1) drawing for each calendar month of Sweepstakes Period, for a total of 12 monthly drawings, and a total of 12 prizes. Each prize consists of a $50.00 Visa gift card. (Total value of all prizes is $600.) Odds of winning each prize depend on number of eligible entries received during the calendar month immediately preceding the applicable drawing. Answers to the survey and method of entry have no effect on your ability to participate or odds of winning. The non-winning entries in a given calendar monthly drawing will not be carried forward to succeeding monthly drawing pools.
7.Prize Restrictions
All prizes will be awarded, and awarded by gift card delivered via mail or courier within 30 days of Sponsor’s receipt of Required Documents listed in Rule 8 and determination that the potential winner is eligible and in compliance with these Official Rules. Prizes are non-transferable. Limit one (1) prize per entrant.

Potential winners of all prizes will be selected by Sponsor at its office in random drawings held on or about the fifth business day of the month succeeding each calendar month of the Sweepstakes Period; the first drawing will be on November 7, 2011 and the last on October 5, 2012. Decisions of the judges are final. Potential winners will be notified by email, overnight mail or courier, and will be required to complete and return, within 14 days, an affidavit of eligibility, a liability release and, where lawful, a publicity release (collectively, “Required Documents”). If a) prize notification is returned as undeliverable, b) Required Documents are not timely returned, or c) potential winner is found to be ineligible or not in compliance with these Official Rules, potential winner will forfeit all rights to win and be disqualified, and prize for that applicable monthly drawing will be awarded to an alternate by random drawing from remaining applicable pool of eligible entries (i.e., the entries received the same calendar month from which the disqualified entry was drawn).
9. Conditions of Participation

Decisions of the judges are final. Potential winner will be notified by email within five (5) business days of drawing, and will be required to complete and return, within 14 days, an affidavit/declaration of eligibility (including disclosure of tax identification number if required by law), a liability release and, where lawful, a publicity release (the “Required Documents”). If potential winner is a resident of Canada, he/she must also correctly answer, without assistance of any kind, whether mechanical or otherwise, a time-limited mathematical skill-testing question administered online or offline in order to be declared a winner. If Sponsor is unable to contact potential winner by email, or if potential winner fails to respond to the prize notification (and, in the case of a resident of Canada, if the potential winner does not correctly answer the skill-testing question) within 14 days of issuance of notification, or if any potential winner fails to comply with these Official Rules, including submission of the Required Documents, potential winner will forfeit all rights to win and be disqualified and prize will be awarded to an alternate potential winner selected by random drawing conducted by the judges from remaining eligible entries.
